What is Patient Fusion?

Patient Fusion offers personalized and secure online access to portions of your medical record. It enables you to securely use the Internet to help manage and receive information about your health. With Patient Fusion, you can use the Internet to:
  • View test results.
  • Request or cancel medical appointments, and review the date, time, and address of future appointments.
  • Communicate electronically and securely with your medical care team.
  • See blood pressure readings.
  • View your health summary from the Patient Fusion electronic health record.
  • View After Visit Summary of previous visits with your healthcare provider.
  • View recommended preventive care procedures based on age, sex and medical history, and request an appointment for health maintenance follow-up.
  • View the Care Plan your healthcare team has on record for you.
  • Access trusted health information resources.
  • Download visit and health summary information to take with you to other providers or for your own records.

When can I see my test results in Patient Fusion?
Your test results are released to your Patient Portal account within three (3) days after they are available in your electronic health record. Not all test results are released to Patient Portal. For more information, please ask your provider.

If some of my health information on Patient fusion is not correct, what should I do?
Your Patient Portal information comes directly from your electronic medical record at your provider’s office. For some information (such as allergies) you may review and mark for deletion any allergies that do not apply and add any allergies that are not listed. You may also ask your provider through Patient Fusion Messaging or during a visit to correct any inaccurate information, indicating what is inaccurate. Your health information is reviewed and updated in your electronic medical record each visit.

If I send a message to my provider or nurse, when can I expect a reply?
You will generally receive an answer within three (3) business days. Please note that Patient Fusion should not be used for urgent situations. Please contact your provider’s office if the situation requires immediate attention or dial 911 if it is an emergency.

Can’t I cancel or reschedule my appointment through Patient Fusion? 
Yes, to Reschedule or Cancel an appointment, click on Appointments and choose Reschedule or Cancel. There is no fee charged to cancel the appointment. Appointments can also be cancelled by calling the clinic.
